Phone Calls |

| Our Triage Person is available Monday — Friday to answer
baby care questions, medication questions or to help you decide
if your child needs to be seen. If you wish to speak to the
doctors, non-emergency calls are answered at the end of the
afternoon and, as time permits, between appointments. For a
complicated problem, please schedule an appointment. When
calling about a sick child, always have a pencil and the number
of your pharmacy ready. As we have limited staff
on Saturday, please hold all routine calls until the weekdays. |

Medication Refills |

| In general, please have your pharmacy call our office for
medication refills. When calling, please specify dose, time
given, and quantity desired. Please allow 1—2 days for processing. |

Payments |
| |
| Our office policy is to collect payment in full for
services at time of the visit, including all copays due. We
are pleased to accept cash, check, MasterCard or VISA
for your convenience. If you need to arrange a payment plan,
please contact our billing department. |

Fees |
| |
| Charges for professional visits, procedures, medications,
and lab tests are determined based on the time spent, the cost
of supplies and the complexity of the problem. Scheduled consultations
or well visits cancelled with less than 24 hours notice will
be charged to you; missed appointment charges are not
covered by insurance plans. |

| Claims will be submitted to all insurance companies we
are contracted with. |

After Hours |
| |
| For urgent problems that can’t wait until the next day (no
medication refills or routine questions, please!), call the
office number and the answering service will contact the nurse
or doctor-on-call. When you call, describe the problem and
indicate whether you need your call returned immediately or
whether it can wait an hour. |
